2008 Match Play Results Reporting Form

To send the results of your match play directly to the Men's Club Executive please fill in the form below and click on the submit button at the bottom of the form.  Your results will be emailed directly to the Men's Club Webmaster and the website will be updated within 24hrs.  Also please add your name or team to the appropriate place on the match play ladder sheets in the Men's Locker Room.

Your results are not official until they have been confirmed and posted to the website.

  8. Click on the "Submit" button at the bottom of this form. If you make a mistake click on the "Reset" button and start over.
  9. Please check the Hazelmere Men's Club website to make sure that the results were posted correctly.  If you find any errors or omissions (names spelt wrong) please contact hmc@hazelmeremensclub.org immediately.
